Calcified Fragment XXI / 21 - an incision

From the Mausoleum, head towards The Founts (the passage is in the far left corner, assuming the ship is behind you). At the corner, you have to turn, but ahead is a gate, where the Fragment is sitting just behind.

Calcified Fragment XXII / 22 - The High War

From the Mausoleum, head into the Founts. After turning the corner (just after Fragment XX / 20), it's high above on the right. To reach it, climb the structure directly ahead as you come out, then over by the hanging object, and jump across.

Calcified Fragment XXIII / 23 - fire without fuel

Head into The Founts from the Mausoleum, dropping down into the area below. Turn the corner here right, and at the end of this passage is a small crack in the wall you can crawl through. But instead of doing that, go left into the corner, where the Fragment is hidden.

Calcified Fragment XXIV / 24 - THE SCREAM

This is only available in the Regicide mission or The Sunless Cell Strike.

Start going down the left steps when you first enter The Asylum, where you'll see a long pllar into the abyss. From here you should be able to see the Fragment, and it's safe to walk down the pillar to go collect it.

Calcified Fragment XXV / 25 - Dictata ir Dakaua

This is only available in the Regicide mission or The Sunless Cell Strike.

Go down the left steps when you first enter The Asylum, and clear out nearby enemies, since you'll need to do a spot of precision platforming. Looking over the left side near the bottom of the steps, and pop your Ghost, so you can see an invisible platform directly below. Drop down onto it. Turn around so you're facing under the stairs, and pop the Ghost again, where you'll see another invisible platform in the middle. Hop onto it, and turn towards the steps, where you'll see a ledge with the Fragment on.

Calcified Fragment XXVI / 26 - star by star by star

This is only available in the Regicide mission or The Sunless Cell Strike.

Play the Strike to reach The Asylum. After opening the door in the opening area, and just beyond the next door after the arena fight, you'll come to a curved path that's flanked by a multitude of what looks like hanging lanterns. Before you run down the path, look to the left, where there is a ledge with a Fragment.

Calcified Fragment XXVII / 27 - Eat the Sky

This is only available in the Regicide mission.

Play the mission until you travel through the portal. Once through, immediately turn around and look behind one of the tombstones to find the Fragment.

Calcified Fragment XXVIII / 28 - King of Shapes

This is only available in the Shield Brothers Strike.

When you leave the crashed ship, but before you take the passage to the Shield Brothers fight, you're in an open area filled with debris. The Fragment is under what looks like a fallen walkway in the far right corner, hidden under the floor.

Calcified Fragment XXIX / 29 - Carved in Ruin

This is only available in the King's Fall Raid.

After the ship platforming section in Portico, you have to jump into a column of energy to take you to the Basilica, where you have to split into two teams and open the door to the Warpriest fight. Before you go unto this column of energy and go up, there is a pillar in the middle of the room. The Fragment is embedded into the right side.

Calcified Fragment XXX / 30 - a golden amputation

This is only available in the King's Fall Raid.

When you first enter the Basilica, where you have to split into two teams and open the door to the Warpriest fight - as soon as you enter the Fragment is at the base of a pillar to the right.
